520.3.#.a: Empyema necessitatis is a rare long-term complication of thoracic empyema characterized by pus dissecting through soft tissue and skin of the chest wall. the pus communicates with the outside, forming a fistula between the pleural cavity and the chest wall skin. the most common etiological agent is mycobacterium tuberculosis and actynomices israelii. the inicial treatment for this kind of empyema includes broad spectrum antibiotics besides the surgical drainage. the case of a 47 year old male patient, was reported and presented diabetes and chronic alcoholism. he was injured on the right side hemithorax. there also the presence of a pleural burst blood vessel . a ct scanner showed the hemorrhage had invaded the chest wall. the culture for tuberculosis and the polymerase chain reaction test were negative. the cytology study was not malignant. thoracotomy and fasciotomy surgeries were carried out to drain the empyema and the patient recovered from the empyema necessitatis.|el empiema necessitatis es una rara complicación de un absceso en el espacio pleural pulmonar; se caracteriza por la disección de pus que abre paso a través de los tejidos blandos; formando una fístula entre la cavidad pleural y la piel de la pared torácica. el agente etiológico más identificado es mycobacteium tuberculosis; seguido por actynomices israelii. el tratamiento inicial para este tipo de empiema es con antibióticos de amplio espectro; el drenaje quirúrgico es muy importante. reportamos el caso clínico de un paciente del género masculino de 47 años de edad, diabético y alcoholismo crónico. recibió un traumatismo en el hemitórax derecho. se integró un síndrome de derrame pleural y por tomografía se demostró que la colección invadió la pared torácica. los cultivos para tuberculosis y la prueba de reacción en cadena de polimerasa (pcr, por sus siglas en inglés) resultaron negativos. la citología fue benigna. se realizó toracotomía abierta con fasciotomía para drenar el empiema; el paciente recuperó su salud
